A company has been fined £40,000 after releasing a cloud of toxic chlorine gas that spread through its factory, yard and surrounding area.
The incident resulted in staff needing hospital treatment, while there was also significant damage to the factory.
HSE’s investigation discovered that the incident happened because:
– a dedicated mixing plant had not been brought back into service after maintenance work
– the company had failed to introduce effective records management for the temporary manual system

Legislation to allow a wider range of healthcare professionals to certify fit notes has recently been introduced by the UK government.
As of 1 July 2022, nurses, occupational therapists, pharmacists, and physiotherapists have all been able to legally certify fit notes in addition to doctors.
This has made it easier to get advice certified by the most relevant healthcare professional.
The legislation does not alter the purpose and function of the fit note. It will continue to support and empower better conversations about work and health between employers and workers.
These professionals should be working within general practice or delivering NHS services because access to the new version of the fit note is limited to these settings.
