Resources and Referrals
Do you need help navigating the maze of services and programs for older adults? Do you have a question or issues but don’t know where to begin? One of our program coordinators will be available to assist you! Areas of assistance include: referrals to community resources to help with your needs, form completion, making calls to social service agencies, linking to housing, health, legal services, and more.
Assistance is available at South Natomas, Hart Senior Center, and Oak Park Community Center!
Oak Park Community Center: Appointments available every Wednesday from 10a-1p. To schedule an appointment call (916) 808-6151.
South Natomas Community Center: Appointments available every Thursday. Please call to see appointment availability at (916) 808-1571.
Hart Senior Center: Appointments available every Monday. Please call to see appointment availability at (916) 808-5462.
The Hart Senior Fun Pass is an income-based, fee-waiver supported by the Hart Trust Fund for Senior Citizens. Complete the application and, if determined eligible, receive a $100 credit toward classes and programs offered by the City of Sacramento’s Recreation Division.
To be eligible for the Hart Fun Pass Program, you must be age 50 or above and a resident of the City of Sacramento. To download the Fun Pass application, click the link below!
