Manufacturing company fined after worker fatally crushed [ https://press.hse.gov.uk/2024/03/05/manufacturing-company-fined-after-worker-fatally-crushed/?utm_source=govdelivery&utm_medium=email&utm_campaign=press-channels-push&utm_term=fatal-crush-headline&utm_content=digest-7-mar-24 ]
The £120,000 fine was issued after a worker was killed when he was trapped between 2 heavy steel beams.
The worker was using an overhead travelling crane to lift the middle of 3 beams to position it better. As it was lifted the 1,800 kg beam rotated in a sling, toppling sideways and trapping him against another beam, causing fatal injuries.
HSE’s investigation found the company:
* failed to ensure a suitable and sufficient risk assessment was carried out by a competent person to identify well known industry standard control measures
* did not properly plan lifts
* did not have a system for ensuring that there were adequately qualified supervisors present during lifting operations
